AGREEMENT TO ASSUME RISK AND RELEASE FROM LIABILITY

COVID-19 WAIVER

 

Full Contact Sports Activity

 

I, the undersigned, being aware of my own health and physical condition, and having knowledge that my participation in any contact sports Activity or program may be injurious to my health, am voluntarily participating in a full contact physical Activity with Jacksonville Roller Derby, LLC. I acknowledge and understand that I have a previous or existing injury and wish to participate without a written clearance from a medical professional, against the Policies & Procedures of Jacksonville Roller Derby.

 

I understand and acknowledge that there are risks, including physical injury, disability, illness, or death as a result of allowing myself to participate in this activity. I agree to disclose any physical limitations, ailments, or impairments which may affect my ability to participate in said physical activity. I accept responsibility for any injury, illness, or ailment that occurs as a result of my participation in this full contact sports Activity. I accept responsibility for all medical bills, co-payments, and deductibles and will not seek reimbursement from Jacksonville Roller Derby. I agree to waive any and all claims that I may have, arising out of or in any way connected with the Activity.

 

I am aware of the COVID-19 pandemic and related governmental orders, directives and guidelines (collectively “directives”), including directives for frequent hand washing, social distancing and use of face masks in public locations. I am aware that Jacksonville Roller Derby activities are occurring in a public location during the COVID-19 pandemic, and are therefore hazardous activities. I am aware that I could be infected, seriously injured or even die due to COVID-19 or due to activities I engage in with Jacksonville Roller Derby.

 

In adherence to JRD Policies & Procedures guidelines, all participants attending practices are required to abide by the following code of conduct. This code of conduct extends to all parties who attend practice regardless of participation level including partners/friends/children of participants. Failure to abide by the code of conduct will result in immediate loss of attendance privileges and potential ban from future JRD events.

 

JRD Drop-in Practice Code of Conduct:

Attendees will engage in personal conduct that is conducive with growing/maintaining a league culture that promotes respect and dignity for all. Attendees will abide by the WFTDA policies regarding Diversity & Inclusion, Gender, and anti-harassment. Policies can be found here: https://resources.wftda.org/membership/diversity-and-inclusion/

Any concerns regarding respect and dignity, harassment, or the conduct of others should be discussed (either verbally or in writing) to the training team within 24 hours of the incident.

Attendees will abide by JRD policies related to Public Health & Covid-19 including refraining from attending any practice if experiencing symptoms of illness and/or tests positive for Covid-19/Influenza etc. Wear masks when required. Notify the BOD immediately if attendee tests positive within a week of attending a practice.

 

Having such knowledge, I hereby acknowledge this document, and release any representatives, agents, and successors of Jacksonville Roller Derby, LLC. from liability related to injury or illness I may incur as a result of participation in said physical Activity. I hereby assume all risks connected therewith and consent to participation in said Activity. This release and waiver is binding on my heirs, assigness, and representatives.
